

Information for New Clients of Willow Animal Hospital
If you haven’t scheduled an appointment for your pet with our animal hospital, please do so now. And if you have, then welcome! We are thrilled at the opportunity to serve another special pet with personalized, caring attention.
Please note that Willow is happy to work with your online pharmacy, such as Chewy, but requires 3-5 business days to process requests and refills.

What You’ll Need to Know
- Patient Check-In Sheet for new patients (just print and bring with you–If you are unable to print and fill out in advance, please arrive 15 minutes before your appointment so you can fill it out at the Hospital.)
- Please note that a receipt from your previous vet does not constitute “medical records.”
- Prescription Refill Request form to make reordering easy (PLEASE NOTE: Willow needs up to 5 business days to process Chewy/On-Line pharmacy refills!)
- Contact Form you can use for questions and concerns

Please note: Willow Staff will send your test results, appointment information, and other digital documents to the email for you that we have on file. Please ensure that we have the correct email, and that emails from us at willowvethospital.com or evetpractice1.com are not landing in your spam folder!
If you need to have anything faxed to the hospital, please use: (540) 216-0959, or you can give your previous vet our email: willow@willowvethospital.com to send information.
